Responsibilities

  • Provide evidence-based physical therapy assessments and treatments to clients.

  • Develop and implement personalized therapy plans with measurable milestones.

  • Maintain up-to-date clinical skills through ongoing professional development.

  • Monitor patient progress and adjust plans to optimize outcomes.

  • Educate patients and families on exercises, injury prevention, and self-management.

  • Document all sessions accurately in electronic medical records.

  • Meet or exceed individual performance metrics and targets.

  • Ensure compliance with safety, privacy, and regulatory standards.

  • Collaborate with multidisciplinary team to coordinate comprehensive care.

  • Identify opportunities for process improvement and contribute to growth initiatives.

  • Engage in on-site visibility activities to showcase impact and progress.


Experience/Qualifications

  • DPT or relevant PT license with current state licensure.

  • Evidence-based assessment and treatment planning for varied client groups.

  • Develop, implement, and track measurable therapy milestones and goals.

  • Safety, privacy, and regulatory compliance in clinical practice.

  • Commitment to ongoing professional development and competency growth.

  • Strong documentation skills in electronic medical records (EMR).

  • Regular progress monitoring and plan adjustments to optimize outcomes.

  • Patient and family education on exercises, prevention, and self-management.

  • Collaborate with multidisciplinary team for integrated care planning.

  • On-site visibility and outreach to demonstrate impact and progress.
