Responsibilities
Provide evidence-based physical therapy assessments and treatments to clients.
Develop and implement personalized therapy plans with measurable milestones.
Maintain up-to-date clinical skills through ongoing professional development.
Monitor patient progress and adjust plans to optimize outcomes.
Educate patients and families on exercises, injury prevention, and self-management.
Document all sessions accurately in electronic medical records.
Meet or exceed individual performance metrics and targets.
Ensure compliance with safety, privacy, and regulatory standards.
Collaborate with multidisciplinary team to coordinate comprehensive care.
Identify opportunities for process improvement and contribute to growth initiatives.
Engage in on-site visibility activities to showcase impact and progress.
Experience/Qualifications
DPT or relevant PT license with current state licensure.
Evidence-based assessment and treatment planning for varied client groups.
Develop, implement, and track measurable therapy milestones and goals.
Safety, privacy, and regulatory compliance in clinical practice.
Commitment to ongoing professional development and competency growth.
Strong documentation skills in electronic medical records (EMR).
Regular progress monitoring and plan adjustments to optimize outcomes.
Patient and family education on exercises, prevention, and self-management.
Collaborate with multidisciplinary team for integrated care planning.
On-site visibility and outreach to demonstrate impact and progress.
