Setting: In-house Subacute Rehabilitation Facility

Position Summary:
We are seeking a skilled and compassionate Per Diem Physical Therapist (PT) to join our in-house subacute rehabilitation team. The PT will evaluate and treat residents to improve mobility, strength, balance, and overall functional independence, while promoting safe discharge planning and optimal outcomes.

Key Responsibilities:

  • Perform comprehensive physical therapy evaluations and develop individualized treatment plans
  • Provide therapeutic interventions including exercise, gait training, balance training, and functional mobility activities
  • Supervise and direct Physical Therapist Assistants (PTAs) as appropriate
  • Monitor patient progress and modify treatment plans to achieve goals
  • Collaborate with interdisciplinary team members including OT, SLP, nursing, and physicians
  • Educate residents and caregivers on treatment plans, safety, and home exercise programs
  • Complete accurate and timely documentation in compliance with regulatory and facility standards
  • Participate in care planning and discharge planning

Qualifications:

  • Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
  • Current Nebraska Physical Therapist license (or eligibility)
  • Experience in skilled nursing or subacute rehabilitation preferred
  • Strong clinical, communication, and organizational skills
  • Ability to work independently and as part of a team

Schedule:

  • Per diem / PRN (as needed)
