    Job Summary 
     As a home health Physical Therapist Assistant, you will work with patients in the comfort of their own homes, providing personalized care to help them recover from injury or illness. You will provide skilled therapy services to improve quality of life and promote independence in the home setting. 
    Job Responsibilities 
    ·        Provide therapeutic intervention following treatment plan established by Physical Therapist 
    ·        Promote independence in the home setting through therapeutic intervention and modify treatment plan as needed 
    ·        Educate patients, families, caregivers regarding home exercise program, safety, falls prevention 
    ·        Communicate effectively with all team members 
    ·        Maintain patient records in accordance with federal and state regulations and    company policy to ensure quality standard of care. 
    Education 
    ·        Associates Degree Physical Therapist Assistant 
     Required Certification 
    ·        California Physical Therapist Assistant license 
    ·        Current driver’s license 
    ·        Automobile insurance 
    ·        Current CPR/BLS-Healthcare Provider 
    ·        Current Physical/TB
