As a compassionate and dedicated Physical Therapist Assistant (PTA), you will play a vital role in helping patients achieve their rehabilitation goals and improve their quality of life. Working under the direction and supervision of a licensed Physical Therapist, you will provide coordinated care to patients of all age groups. Duties and Responsibilities:
- Provide physical therapy treatments and interventions as outlined in the Plan of Care established by the supervising Physical Therapist.
- Observe, monitor, and document patient responses to treatment and communicate findings to the Physical Therapist.
- Assist patients in improving mobility, strength, balance, endurance, coordination, and functional independence.
- Instruct patients and caregivers on prescribed therapeutic exercises, home exercise programs, and safety techniques.
- Recognize changes in patient condition and promptly report concerns to the supervising Physical Therapist and care team.
- Maintain accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
- Assist with evaluating and recommending durable medical equipment and adaptive devices as directed by the Physical Therapist.
- Apply infection control principles to prevent contamination and ensure patient safety.
- Participate in mandatory continuing education and in-service programs.
Required Skills/Abilities:
- Strong therapy skills and knowledge of accepted physical therapy practices.
- Excellent interpersonal and communication skills.
- Ability to work effectively under the supervision of a licensed Physical Therapist.
- Knowledge of durable medical equipment and assistive devices.
- Strong organizational and documentation skills.
- Ability to work a flexible schedule and travel locally.
- Reliable transportation and valid auto liability insurance.
Required Education and Experience:
- Graduate of an accredited Physical Therapist Assistant program.
- Current Texas State License as a Physical Therapist Assistant.
- Minimum one (1) year of experience in home health, acute care, rehabilitation, or a related healthcare setting preferred.
- Current CPR certification preferred.
Physical Requirements:
- Must be able to lift and/or move up to 50 pounds.
- Frequent standing, walking, bending, reaching, and hand dexterity required.
- Ability to assist patients with transfers, mobility activities, and therapeutic exercises.
- Close vision, color vision, and depth perception abilities needed.
Work Location & Travel Requirements:
- Home office location is MyCHN Home Health Services:4005 Technology Dr, Angleton, TX 77515, USA
- This position requires regular travel to patient homes and other assigned locations throughout Brazoria, Galveston, and Harris Counties.
