You will:
• Help patients regain strength, balance, and mobility • Support recovery after illness, surgery, or hospitalization • Reduce fall risk and prevent re-hospitalizations • Carry out therapy plans developed by the Physical Therapist • Help patients remain safely in their homes

What You'll Be Doing
• Providing physical therapy treatments in patients' homes • Following care plans established by the Physical Therapist • Monitoring patient progress and reporting changes • Educating patients and caregivers • Documenting visits and collaborating with the care team
What Helps You Succeed
• Graduate of an accredited Physical Therapist Assistant program • Licensed PTA in the State of Oregon • Strong communication and organizational skills • Ability to work independently in the field • Valid driver's license and reliable transportation
Home health experience preferred but not required.
