Pay Range 28.08hr. to 38.09hr. hourly non-exempt based on qualifications, experience, and location.

As a PTA, youll play a vital role in helping residents and patients maintain their highest level of function. Youll implement treatment plans under the direction of a Physical Therapist, assist with mobility and rehabilitation exercises, and contribute to a collaborative care team focused on improving quality of life.
As a PTA, you will: • Deliver Individualized Therapy Care - Assist with resident screenings, implement treatment programs designed by the Physical Therapist, and adjust therapies based on patient response. Support mobility, strength, and rehabilitation efforts using exercises, assistive devices, and other techniques. • Document and Track Progress - Document all services accurately and timely using Vivies EMR system. Maintain accurate, timely records of treatments, progress, and patient responses. Complete billing documentation and ensure compliance with medical record guidelines. • Collaborate with Care Teams - Work closely with Physical Therapists, physicians, case managers, and families to optimize patient outcomes. Participate in care planning meetings, provide progress updates, and recommend discharge plans. • Ensure Safety and Compliance - Maintain equipment, follow infection control protocols, and uphold therapy standards. Stay updated on best practices and participate in professional development opportunities. • Communicate Effectively & Collaborate - Engage with clients, caregivers, and family members to set meaningful functional goals. Provide clear and compassionate updates on client progress while maintaining confidentiality. Collaborate with the interdisciplinary team to ensure a seamless and client-centered approach to care. Recommend follow-up programs for outpatient or home-based therapy. • Other Duties as Assigned - The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.
Requirements • Associate degree from an accredited Physical Therapy Assistant program. • PTA license in the state of Minnesota required. • CPR certification or willingness to obtain within 6 months of hire. • Understanding of medical documentation, billing processes, and compliance regulations. • Experience in home health, skilled nursing, or rehabilitation settings is preferred but not required. • Valid drivers license and access to an insured personal vehicle. • Excellent communication and teamwork skills to collaborate effectively with interdisciplinary teams. • Strong knowledge of physical therapy practices and rehabilitation techniques. • Ability to pass state-mandated background check. • Physical capability to perform all essential job functions. • Ability to read, write, and speak English to ensure effective communication with staff, residents, and families.
