As a Home Health PTA, you’ll work under the direction of a Physical Therapist while providing one-on-one care, supporting individualized treatment plans, and helping patients reach their therapy goals.

What You’ll Do
Provide skilled therapy services in patients’ homes under the direction of a Physical Therapist
Assist patients with exercises, mobility, strengthening, and functional goals
Support treatment plans focused on restoring independence and improving quality of life
Educate patients and families on therapy techniques, safety, and equipment use
Observe and report patient progress, changes, and concerns to the Physical Therapist
Collaborate with physicians, therapists, and the interdisciplinary care team
Complete timely clinical documentation in accordance with agency and Medicare guidelines
Participate in case conferences, trainings, and on-call requirements as needed
Qualifications
Current Physical Therapist Assistant license in the state of employment
Minimum of 1 year of PTA experience in an acute care setting required
Home health experience preferred
Valid driver’s license and auto insurance
Strong communication, time management, and documentation skills
Compassionate, dependable, and comfortable working independently in the field
