Physical Therapist Assistant | PRN Float | Nashville, TN
All Heart Home Care is seeking a dedicated and compassionate Physical Therapist Assistant (PTA) to join our home health team as a PRN Float in the Nashville area. This flexible, as-needed role offers the opportunity to provide personalized, one-on-one care to patients in the comfort of their homes while supporting multiple locations as needed.
Key Responsibilities:
- Provide in-home physical therapy services under the direction and supervision of a licensed Physical Therapist
- Assist in implementing individualized treatment plans to improve patient mobility, strength, balance, and overall function
- Monitor patient progress and report any changes to the supervising Physical Therapist
- Educate patients and caregivers on prescribed exercises, safety protocols, and home care techniques
- Maintain accurate and timely clinical documentation in compliance with company policies and regulatory requirements
- Collaborate with the interdisciplinary team to ensure coordinated and effective patient care
- Travel to patient homes throughout the Nashville service area as needed
Qualifications:
- Graduate of an accredited Physical Therapist Assistant program
- Active Physical Therapist Assistant license in the state of Tennessee
- Current CPR certification
- Valid driver's license, reliable transportation, and current auto insurance
- Strong communication and interpersonal skills
- Ability to work independently and adapt to varying patient needs and locations
- Previous home health experience preferred but not required
Work Schedule:
- PRN (as needed)
- Flexible scheduling based on patient needs and coverage demands
