Position Title: Physical Therapist Assistant Fremont, CA
Location: Fremont, CA
Compensation: $45-$50/hour
Schedule: Monday-Friday, 35 hours/week, no weekend rotation
Setting: Skilled Nursing Facility (SNF) / Post-Acute Rehabilitation Center / Nursing Home

A post-acute rehabilitation provider delivering skilled nursing and therapy services in a collaborative care setting.

Position Summary / Responsibilities
• Assist in carrying out physical therapy treatment plans
• Document patient progress and communicate updates to supervising therapists
• Support patient mobility and rehabilitation goals

Requirements
• Current Physical Therapist Assistant license
• Experience in SNF or rehabilitation settings preferred
• Strong teamwork and communication skills
