Job Summary and Qualifications:

We are currently looking for an ambitious Medication History Pharmacy Technician. The Pharmacy Medication History Technician performs medication histories for new hospital admission. You will under general supervision of the clinical pharmacist, provide complete and accurate information for each medication, e.g., drug name, dose, route, frequency, indication, and date/time last dose was taken.Join us in our efforts to better our community!

What you will do in this role:

You will perform medication histories for new hospital admission.

You will utilize available resources for medication history collection including but not limited to: patient/family interview, medication lists, outside facility medication administration records, outpatient pharmacy records, and physician office records.

You will verify and update patient allergy, adverse drug reaction, preferred pharmacy and primary care physician information in the electronic medical record. 

Required Job Qualifications include:

  • High School Graduate or GED required
  • 2 years previous retail pharmacy experience required
  • 1-year previous inpatient pharmacy experience preferred
  • Maintains current pharmacy technician registration or pharmacy intern license required.
  • Certification as a Pharmacy Technician for registered pharmacy technicians; a 90-day grace period is allowed after hiring in the role required.

This role requires you to be fully vaccinated for COVID-19 based on local, state and/or federal law or regulations (unless a medical or religious exemption is approved).

What qualifications you will need:

  • (PharmTech) Pharmacy Technician License
  • High School Graduate / GED
  • (CPhT) Certified Pharmacy Technician, or (PHARM-Intern) Pharmacist - Intern
