Job Summary
Under the direction of the Clinical Pharmacy Manager and ED Pharmacists oversight, the Pharmacy Medication Reconciliation Technician is responsible for obtaining a comprehensive and accurate patient home medication list as part of the admissions process. This role will have a significant impact on improving safety and quality of our patients by providing physicians and other clinicians with a timely comprehensive home medication list.
Coordinates/assists with deliveries of code trays to the Emergency Department Actively participates in ongoing quality improvement programs focused on medication reconciliation.

High School diploma required Must have one year of Pharmacy Technician experience Familiarity with medical/drug terminology and abbreviations. Demonstrated ability to walk and stand 70% of the time and to lift up to 35 pounds Must be ExCPT or PTCB certified, or a registered pharmacy intern with the state of Massachusetts Must be Registered with the Massachusetts Board of Pharmacy as a Pharmacy Technician or Intern Excellent communication and computer skills Physical Environment: Satisfactory physical environment.
