As a Pharmacy Technician – Care Manager, you will work closely with healthcare teams and patients to optimize outcomes and improve overall health.
Responsibilities (RE)
- Conduct medication reconciliation and resolve discrepancies with care teams
- Educate patients on medication use, side effects, and adherence strategies
- Monitor medication adherence and follow up with at-risk patients
- Support care transitions, including refills, prior authorizations, and delivery coordination
- Maintain accurate documentation in EHR systems
- Collaborate with pharmacists to identify drug interactions and improve care plans
- Ensure compliance with HIPAA and pharmacy regulations
- Participate in quality improvement initiatives
Requirements
Qualifications
- Certification: Current Pharmacy Technician License from a recognized certifying body (e.g., PTCB or NHA).
- Experience: Previous experience in a pharmacy setting, ideally with a focus on care management or chronic disease management.
- Knowledge: Strong understanding of pharmacology, medication therapy management (MTM), and patient-centered care practices.
Skills
- Technology Proficiency: Familiarity with EHR systems, pharmacy software, and telehealth tools.
- Communication: Excellent verbal and written communication skills for patient and provider interactions.
- Attention to Detail: Exceptional accuracy in documenting patient information and managing medication orders.
- Problem-Solving: A proactive approach to identifying and resolving medication-related issues.
