The MetroHealth System is redefining health care by going beyond medical treatment to improve the foundations of community health and well-being: affordable housing, a cleaner environment, economic opportunity and access to fresh food, convenient transportation, legal help and other services. The system strives to become as good at preventing disease as it is at treating it. Founded in 1837, Cuyahoga County's safety-net health system operates four hospitals, four emergency departments and more than 20 health centers.

Summary:

Provides direct patient medication-related care services and comprehensive pharmacy navigation to improve patient outcomes and support value-based care delivery. Serves as a clinical pharmacist fully integrated into daily ambulatory clinic operations and co-located with the care team. Focuses on medication therapy management, medication access and coverage navigation, prior authorization coordination, disease management, population health activities, and patient adherence support. Makes medication-related decisions on drug therapy regarding selection, dosages, routes, methods of administration, medication therapy monitoring, and provision of medication-related information and counseling to individual patients in assigned clinical areas. Works with physicians and other healthcare professionals, with CPA, to optimize medication use through chart reviews, evidence-based recommendations, quality monitoring, and improvement initiatives that support CMS Star Ratings and value-based payment performance. Consults with the patient and physician(s) to identify, resolve and prevent medication related problems, as well as establish mutually agreed upon treatment goals.
