    Description

    Reporting to the Senior Program Director, the Registered Nurse is responsible for providing medical support and supervision for the elderly program and its participants. The Registered Nurse will also provide training to staff and referral information and education to participants.

    ESSENTIAL DUTIES AND RESPONSIBILITIES

    • Participates in referral, medical assessment, and discharge or termination of clients.
    • Participate in the development, evaluation, monitoring and revision of medical issues pertaining to the individualized rehabilitation plans.
    • Provides consultation and training staff.
    • Participates in the process to privilege staff to perform specific duties.
    • Serves as a liaison and consultant to medical community.
    • Monitors medication policies and procedures.
    • Administers medications, applies/changes dressings, conducts vital sign checks, reviews medical tests and makes referrals for follow-up patient care as required.
    • Monitors client physicals and makes recommendations.

    EFFECT ON END RESULTS:

    • Ensures the monitoring of the medical status of participants.
    • Ensures compliance with contractual and state regulations relating to medical services.
    • Maintains an effective relationship with other community providers to ensure that program participants receive needed medical services.
