365 Healthcare is recruiting a Board Certified Family Medicine Physician for a full-time permanent outpatient opportunity in Saratoga Springs, New York.

Position Snapshot
- Base Salary: $250,000 – $260,000 + productivity incentives
- Sign-On Bonus: Up to $75,000
- Retention Bonus: $25,000 (Year 2)
- Schedule: 4- or 5-day work week
- Hours: 40 hours/week (34 patient-facing)
- Patient Volume: 18–24 patients per day
- Employment Type: Full-time, permanent
Schedule & Work-Life Balance
- Flexible 4- or 5-day schedule
- No in-office weekend hours
- Call: Approximately 1 in every 70 days
- Phone call only
- RN triage handles first contact
- Telemedicine coverage:
- 2–3 hours on a weekend once every 3 months

Practice Environment

Clinical Responsibilities
- Provide comprehensive primary care to patients of all ages
- Manage chronic and acute conditions
- Perform routine exams and preventive care
- Collaborate with APPs and care teams
- Maintain accurate documentation in Epic
- Deliver patient-centered, evidence-based care
Qualifications
- Board Certified or Board Eligible in Family Medicine
- Active New York license (or ability to obtain)
- DEA and ACLS certification (by start date)
- Strong interest in full-spectrum primary care
- Clean malpractice and license history
