Experience/Qualifications

• Licensed Physical Therapist with current state license required.

• Compliance with regulatory guidelines, safety protocols, and ethics.

• Demonstrated on-site physical therapy treatment delivery per plan of care.

• Accurate documentation of treatments and outcomes in EHR within 24 hours.

• Regular progress assessments; adjust plans and record rationale clearly.

• Interdisciplinary collaboration for care coordination and discharge planning.

• Strong communication skills to educate patients and families on home programs.

• Commitment to ongoing professional development and certifications.

• Productivity targets with quality-of-care balance; track performance metrics.

• Proactive participation in process improvements and performance reviews.
