Are you a highly skilled and compassionate Social Worker looking to make a difference in the lives of patients and their families facing end-of-life care? We are seeking a talented and motivated Social Worker to join our specialized team in providing comfort and support to those in need. We are dedicated to exceeding the expectations of our patients and their families by embracing excellence in all that we do.

Section 1: Overview

• Implement standards of care for medical social work services
• Assess the psychosocial status of patients and families/caregivers
• Carry out social evaluations and provide counseling as needed
• Maintain clinical records and provide information and referral services

Section 2: Qualifications

• Graduate of a master's program in social work accredited by the Council on Social Work Education
• Minimum of one year's experience in health care, hospice experience preferred
• Good verbal and written communication skills
• Possess and maintain current CPR Certification
