Job Responsibilities
- Complete initial client assessments and admission visits (SOC).
- Responsible for performing initial and ongoing competency assessments for Home Health Aides (HHAs) to ensure safe, compliant patient care.
- Report client health status clearly and promptly to the Nurse Supervisor for proper care coordination
- Travel throughout Broward County as needed
- Demonstrate empathy, professionalism, and respect when working with clients and families
- Ensure all documentation is complete, precise, and submitted on time.
- Anticipate potential issues and appropriately escalate or resolve concerns
- Prioritize tasks and demonstrate a strong sense of urgency
Required Experience & Education
- Active Florida Registered Nurse (RN) license
- Current Level 2 clearance in the AHCA Clearinghouse
- TB test within the last 12 months or Chest X-ray within the last 5 years
- Prior home care experience required - Minimum 1-2 years
- Driver's License (Required)
- CPR/BLS Certification (Required)
- Spanish Speaking a plus!
