LEAP TEAM (Linking, Engaging, Advocating, Assessing, and Planning) is a short term to intermediate term multidisciplinary community team designed to engage and treat individuals who often fall through a gap in the community mental health continuum of care. These individuals tend to be high utilizers of acute care mental health systems as well as state mental health treatment facilities. By definition, this population is often difficult to engage in services due to lack of readiness for change and symptom severity/complexity and lack of adherence to treatment. The LEAP team consists of case managers, nurses, peer specialist, counselor, and a physician's assistant for medication management. The team is interdisciplinary and does home visits to meet the individual’s right where they are located.
