Job Summary
"Navitas Healthcare, LLC" is seeking an experienced Pediatric Case Manager RN to join a leading acute care hospital in Anchorage, Alaska. The ideal candidate will have pediatric nursing experience, acute care experience, and strong case management skills to coordinate patient care and support positive outcomes for pediatric patients and their families.
Key Responsibilities
- Coordinate and manage care plans for pediatric patients
- Collaborate with physicians, nurses, social workers, and multidisciplinary teams
- Assess patient needs and develop discharge planning strategies
- Monitor patient progress and ensure continuity of care
- Advocate for patients and families throughout the care process
- Maintain accurate documentation within EPIC
- Facilitate communication between providers, patients, and caregivers
- Ensure compliance with hospital policies and case management standards
- Support utilization management and resource coordination
Requirements
- Active Alaska RN License (Required at Submission)
- Minimum 2 Years of RN Experience
- Pediatric Experience Required
- Acute Care Experience Required
- EPIC Experience Required
- ACM Certification Preferred
- Strong communication and care coordination skills
