Position Statement The Primary Care Patient Services Coordinator coordinates referrals and services for patients and enters patient information to EMR according to established documentation guidelines. Key Responsibilities: At the request of the provider, schedules appointments for patients with other departments within and outside North Country Health System. Gives appropriate instructions to patients for all scheduled appointments and tracks outside appointments to ensure proper follow through. Ensures that documentation for outside referrals is received by referring provider and scanned into EMR, including RCT referrals and MRI screening forms. Confirms outside appointments with patient by phone or face to face. Courteously receives, places and transfers calls using proper phone etiquette, handling phone information requests with accuracy and respect for patient confidentiality. Forwards messages to the appropriate personnel using various forms of communication. Courteously assists patients with completing departmental forms. Files patient information according to established protocol. Participates in analysis of the data entry process to ensure optimization and data accuracy. Knowledge, Skills, and Abilities: High school diploma required. Possesses excellent organizational skills paired with verbal written and communication skills. Demonstrates strong interpersonal skills and capability to work effectively with all staff. Demonstrates excellent customer service skills. Shows competency in computer skills. Understands medical terminology. Transcription experience helpful. One year of experience in a medical practice preferred.

Founded in 1919 by a group of area citizens, Orleans Country Memorial Hospital opened in 1924, the present North Country Hospital, opened in 1974.
