As a PSS-Care Coordinator at P3 Health Partners, you’ll bring hope and healing to patients with complex medical needs. In this full-time role, you’ll play a vital role as a team member, transforming lives and empowering primary care physicians to deliver exceptional care to those who need it most.
As a PSS-Care Coordinator, you will enthusiastically welcome patients and their caretakers as they enter a P3 Health Partner facility developing a sense of trust and confidence in the care they are about to receive. This concierge-like approach level sets the patient’s positive experience throughout their visit. Additional accountabilities include, but are not limited to, answering phones and scheduling patients. The care coordinator portion of the position entails gathering pertinent data from multiple sources prior to the patient’s appointment, allowing the care team to effectively manage the patient at the time of their visit. The care coordinator visit prep extends to all patients in comprehensive care be it an appointment at the facility or at a site external to the facility.
This position will be in a P3 Health Partners facility. This is not a remote position.
