The Heart Failure Center Patient Service Line Navigator provides a single point of contact to patients and the multidisciplinary team through which care is coordinated to achieve optimal patient outcomes. This navigator helps bridge the gaps between hospital-based services interventional procedures and outpatient-based services to ensure the provision of appropriate patient care.

Essential Functions -------------------

• This role will directly support our Heart Failure Center under the Heart & Vascular Service Line.
• The Heart Failure Center Patient Service Line Navigator provides a single point of contact to patients and the multidisciplinary team through which care is coordinated to achieve optimal patient outcomes.
• This navigator helps bridge the gaps between hospital-based services interventional procedures and outpatient-based services to ensure the provision of appropriate patient care.
