Job Title : Patient Portal Liaison
Location : New York, NY 10004 Hybrid
Duration : 2+ months contract (Possible extension )
Education : Associate's Degree
Shift Details : M-F 9am - 5pm, 35hrs/week
Job Description:
- Conduct monthly member outreach calls and ensure HIPAA compliance
- Perform LTSS and SDOH assessments to evaluate member needs
- Coordinate care services including appointments, transportation, and medication support
- Document member interactions accurately in care management systems (e.g., DCMS)
- Identify and escalate clinical concerns to appropriate care managers
- Assist members with care coordination to improve health outcomes and reduce gaps in care.
- Bi-lingual skills are strongly preferred in Spanish
