JOB DESCRIPTION

The Patient Placement Coordinator collects and analyzes data on patient admissions, transfers, and bed availability in specified clinical areas to improve patient care and maximize bed utilization.

ESSENTIAL JOB FUNCTIONS

The Patient Placement Coordinator facilitates throughput from all admission sources via appropriate bed placement. This includes:

• Identifying and assigning beds for all admissions from various access points, such as: • The Emergency Department • Day Surgery • Ambulatory Clinics • Pre-admission and same day admissions • Admissions from home • Transfer from ICU • Interfaculty hospital direct admissions through the Transfer Center

The Patient Placement Coordinator is responsible for facilitating patient assignments and communicates regularly with the Clinical Operations Administrator (COA) to address any bed placement need issues promptly.

Responsibilities:

• Performing data entry of patient accounts, including but not limited to: • Correct admission source • Admit reason • Correct treating service/team • Correct admitting and attending faculty and resident • Correct admission status • Entering admission status orders into the EPIC electronic medical record • Answering calls and providing high-level customer service in all interactions with internal and external customers • Maintaining patient confidentiality regarding access to patient and other clinical information via email, computer, and fax • Addressing concerns of patients and providing service recovery and escalating issues to the appropriate chain of command as needed • Entering bed request and bed assignment information into the ED EPIC Track Board for prompt communication of bed placements • Monitoring census and bed clean status in the Bed Tracking system and coordinating requests for STAT cleans as required • Providing department Business Operations Manager or designee updates regarding placement needs • Being able to cross-function to facilitate interfacility transfers in the absence of the Transfer Coordinator to ensure interfacility transfer referrals and transfers from the Emergency Department are in compliance with EMTALA regulations • Prioritizing and completing all work in an accurate, effective, and efficient manner • Participating in team meetings/activities and supporting the philosophy and goals of the team/department

SALARY

Hourly rate commensurate with experience (work up to 19 hours per week/no benefits)
