Schedule Details/Additional Information:

72 hours per pay period.

7:00a-4:30p. Four 9-hour shifts a week.

Weekends: Every other weekend, rotating Saturday/Sunday.

Holidays: Rotating Holiday Groups.

Pay Range

$41.10 - $61.65

Where You Will Work:

Identify eligible patients from ED or inpatient units based on inclusion/exclusion criteria. Coordinate referrals and documentation with Utilization Management and HAH team.

We're Looking For:

  • Completion of an accredited or approved program in nursing

  • Registered Nurse license issued by the state in which the teammate practices

  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association

  • Similar experience with additional skills in case management or social work, preferred

  • GA RN License

  • Strong communication and technology skills for virtual care delivery

What You'll Do:

  • As a skilled and compassionate RN, you will play a pivotal role in providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures.

  • Using evidence-based practice, clinical decision making, compassion, and skills communication while leading efforts to create the safest patient environment and the best patient experience across the continuum.

Coordinate the patient's care to increase the efficiency and quality of care that is delivered to the patient. This role will start with the time of the patient's diagnosis and will continue through the patient's journey throughout their care. Will be the patient advocate and care for them in a highly supportive and personal manner and also will serve as a liaison to ensure that patients and families receive resources and services as needed.
