Position Overview:
Under general direction, the Patient Navigator Supervisor provides operational and functional oversight for Patient Navigator services across Harris County Public Health clinical sites and mobile units. This position standardizes workflows, ensures documentation quality and service fidelity, coaches and develops staff, monitors performance metrics, and strengthens coordination with clinic teams, co-located programs, and community partners. The Supervisor serves as the program lead for patient navigation operations, promoting timely screening, warm handoffs, closed-loop referrals, and effective resolution of non-medical barriers that affect access to care and continuity of services.
Duties & Responsibilities:
- Supervise, coach, and support Patient Navigators across assigned sites; conduct regular one-on-one check-ins, case reviews, and team meetings to reinforce expectations, build skills, and address performance issues.
- Standardize and monitor navigation workflows, including patient identification, screening processes, referral pathways, warm handoffs, closed-loop follow-up, and documentation requirements across fixed and mobile sites.
- Strengthen and maintain the Community Resource Directory and referral relationships by coordinating with internal programs, HPCC partners, and external community organizations to improve service access and referral quality.
- Review charts, notes, and work queues to ensure documentation quality, timely follow-up, and compliance with program standards, privacy requirements, and operational protocols.
- Monitor and analyze program performance indicators, including screening completion, referral volume, warm handoff rates, service connection outcomes, timeliness, and documentation compliance; prepare summaries and reports for leadership.
- Serve as the escalation point for complex cases, unresolved barriers, cross-site workload balancing, and coordination issues involving clinic operations, co-located programs, or community partners.

Requirements
Education:
- Bachelors degree in Social Work, Psychology, Public Health, Health Administration, or related field.
Experience:
- At least two (2) years of experience in patient navigation, case coordination, community health, care management, or a related health or human services setting.
- At least one (1) year of lead, supervisory, or program coordination experience.
- Experience working with undeserved, low-income, culturally diverse populations.
Licenses/Certifications:
- Valid Texas Drivers License.

- Knowledge of patient navigation, care coordination, community resource linkage, and social drivers of health workflows in a clinical or public health setting.

Position Type and Typical Hours of Work:
