Responsibilities:
- Assess patients' social, emotional, and environmental needs and connect them with appropriate resources and services.
- Serve as a liaison between patients, families, care teams, and community organizations to reduce barriers and support care coordination.
- Guide patients through the continuum of care, from screening and diagnosis to treatment, survivorship, and end-of-life care.
- Provide individualized navigation services to address financial, social, and access-to-care challenges.
- Coordinate support services including transportation, insurance, financial assistance, and community resources.
- Facilitate clear communication by arranging interpretation services and supporting patient understanding during visits and follow-up.
- Assist with scheduling, registration, and completion of forms to ensure a smooth patient experience.
- Support cancer screening programs by coordinating appointments, obtaining orders, and promoting early detection initiatives.
- Conduct patient outreach, education, and follow-up to ensure engagement and continuity of care.
- Document patient interactions and program data while supporting reporting requirements and quality initiatives.
Schedule: This position will work Monday-Friday, 8AM-4PM. May require travel to off-site locations for community events. Events may be other times or days than typical schedule.
- Bachelor's degree in a health services related field required.
- Community patient navigation experience with bilingual interpretation services experience preferred.
- Experience in Oncology preferred.
Licensure, Certifications, and Clearances:
- CPR required based on American Heart Association standards that include both a didactic and skills demonstration component within 30 days of hire.
- Act 31 Child Abuse Reporting with renewal
- Act 33 with renewal
- Act 34 with renewal
- Act 73 FBI Clearance with renewal
