Job Summary:The Patient Navigator will provide care coordination services to members with chronic health conditions. Patient Navigator will act as an intermediary between providers and members - serving in numerous roles such as educator, evaluator, service coordinator, community resource researcher and more.
Responsibilities:
High School Diploma or Associate Degree in Health Care
2-years’ experience in the healthcare, community health, social work, supportive housing, or other
social service interventions
Excellent customer service, organizational, interpersonal and communication skills
Ability to multi-task and work under multiple priorities and deadlines in a fast-paced environment
Proficiency with Word and Excel
$19-$20 an hour
