Summary:

The Patient Liaison acts as the main contact and liaison for family/patient for patients with complex scheduling needs and coordination across transdisciplinary care teams.

Responsibilities:

  1. Acts as the main contact and liaison for patients and their families where patient has complex scheduling needs requiring coordination across transdisciplinary care teams.

  2. Assesses patient needs, determines appropriate services, and directs patient to those who can meet their care needs.

  3. Coordinates appointment scheduling functions and prepares the family/patient for entrance to the department.

  4. Performs point-of-service activities, including visit preparation and patient tracking.

  5. Facilitates patient information and charting.

  6. Mentors and acts as resource for care team in patient scheduling and provider support processes.

  7. Other duties as required

Technical Expertise

  1. Experience in customer service is required.

  2. Experience with medical terminology and information is required.

  3. Excellent communication skills, both verbal and written, coupled with strong listening skills.

  4. Strong organizational skills with the ability to multi-task, as well as anticipate and resolve problems.

  5. Experience working in a healthcare environment is preferred.

  6. Experience working with various levels within an organization is required.

  7. Experience working in Microsoft Office (Outlook, Excel, Word) or similar software is required.

  8. Experience working with a computer-based appointment scheduling system is required.

Education and Experience

  1. Education: High school diploma or equivalent is required.

  2. Licensure: None

  3. Certification: Additional certifications may be required based on the assigned department or unit.

  4. Years of relevant experience: Minimum of one (1) years of relevant experience is required, two (2) years is preferred.

  5. Years of supervisory experience: None
