We’re looking for a caring and organized Patient Liaison who thrives in a people-first environment and is passionate about creating exceptional patient experiences. The Patient Liaison serves as the bridge between patients, families, referral sources and clinical staff ensuring every interaction is professional, seamless and patient-focused. This role is ideal for someone who thrives in a fast-paced healthcare setting, enjoys helping others and brings strong organization, communication and problem-solving skills to the team.

This position supports operations across both our Boca Raton and Pompano Beach locations and reports directly to the Office Administrator.

Responsibilities

  • Be the friendly, supportive connection between patients, families, and our care teams
  • Coordinate patient intake, scheduling, and ongoing communication
  • Build strong relationships with referral sources and healthcare partners
  • Maintain accurate records and ensure HIPAA compliance
  • Help resolve patient concerns with empathy and professionalism
  • Support daily office operations and administrative tasks
  • Assist with insurance verification, referrals, and scheduling processes

Skills and Competencies

  • Home health or healthcare administrative experience
  • Strong customer service and communication skills
  • Experience with insurance verification and referrals
  • Comfortable multitasking in a fast-paced environment
  • Proficiency with EMR systems and Microsoft Excel
  •  Highly organized with strong attention to detail
  •  Compassionate, dependable, and patient-focused

Qualifications

  • College degree
  • Minimum 2 years of customer service experience
  • Minimum 2 years of administrative experience in a healthcare or doctor’s office setting
  • Experience with insurance verification and referrals
  • Proficient computer skills
  • Excellent verbal, written and organizational skills
  • Ability to work independently and collaboratively
