Working with the Case Manager, the Patient Health Advocate is a member of the Care Management team the Health Advocate assists with the daily management of a patient population by collecting detailed information regarding the patient, monitoring care, scheduling preventative and follow-up visits, and assisting the patient with goal setting and the management of their care.

Required Qualifications:

  • Coursework/Training: Completed coursework in Medical Terminology OR working knowledge of Medical Terminology.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • 5 years of related work experience in an acute care or ambulatory care facility, or medical group practice.
  • 1 year Demonstrates knowledge and use of ICD-10 coding

Preferred Qualifications:

  • Associate''s Degree
  • Managed care experience.
  • EPIC EMR experience and competency
  • 1 year Experience in Health medical claims/insurance operations
  • 1 year Experience in Utilization Management
  • 1 year Experience in Quality Measures and HCC Capture

The successful candidate''s placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
