Job Description:
Schedule: Full Time, Hourly
Shift: Monday-Friday with Rotation Supervisor On-Call.
Location: Hospice is based out of our Altamonte Springs Hospice Office
Participates in staff meetings, department meetings, in-services, and other related activities. Other duties as assigned. Develops, implements, and evaluates interdisciplinary team (IDT goals and objectives. Conducts weekly interdisciplinary team meetings to review and ensure patient care, staff relations, and efficiency of service. Investigates, documents, resolves, and reports on problems and issues involving staff or quality of care. Reviews interdisciplinary team documentation and charting to ensure it is timely, accurate, and complete. Acts as a liaison with physicians regarding clinical issues and with family members to address questions and resolve problems. Accompanies staff periodically to evaluate performance and provide guidance to improve skills. Carries out supervisory responsibilities in accordance with organizational policies and applicable laws. Collaborates with Human Resources for employee interviewing, hiring, training, and coaching functions. Oversees and ensures completion of required hospice orientation and training for staff. Schedules and attends meetings with management to market hospice services and increase referrals. Communicates with other departments on subjects including medical records, benefits, finance, case management, and marketing.

$68,132.50 - $119,520.35
