Clinical Leadership & Care Coordination

Schedule: Full-time | Monday–Friday | 8:00 a.m. – 5:00 p.m.

Your Day-to-Day

  • Provide clinical supervision to ensure patient care aligns with professional standards, agency policies, and regulatory requirements
  • Coordinate and monitor the delivery of home health services across assigned patients and care teams
  • Lead, mentor, and support clinical staff to ensure consistent, high-quality patient care
  • Maintain effective communication with patients, caregivers, referral sources, and both field and office personnel
  • Drive quality outcomes by supporting departmental goals and continuous improvement initiatives
  • Serve as a role model for professionalism, accountability, and organizational values
  • Ensure compliance with Medicare requirements, agency policies, and applicable federal, state, and local regulations
  • Complete required training, reporting, and audit-related activities accurately and on time
  • Promote workplace safety by following established protocols and best practices
  • Support employee growth through coaching, mentoring, and continuing education opportunities

What We're Looking For

  • Current Registered Nurse (RN) license in the practicing state
  • Knowledge of accepted professional nursing standards and practices
  • Understanding of Medicare Conditions of Participation and applicable regulatory requirements
  • Strong leadership, communication, and care coordination skills
  • Ability to manage multiple priorities while maintaining a patient-centered approach
  • Home health experience preferred
  • Two years of management or supervisory experience preferred
