The Home Health Patient Care Manager and RN Hybrid is responsible for the supervision and coordination of clinical services and provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations.

This position will spend 50% of their time in the field, and 50% of their time in the office.

Primary Responsibilities:

  • Coordinates referrals, authorizations, start-of-care (SOC) visits, and clinician assignments to ensure timely, appropriate patient evaluations
  • Manages ongoing patient care coordination, including benefits verification, medical necessity, insurance approvals, and payer change documentation
  • Oversees clinical documentation quality and timeliness, including OASIS assessments, visit notes, orders, care plans, and productivity standards
  • Serves as a central communication hub with physicians, clinicians, patients, families, referral sources, and interdisciplinary partners to ensure continuity of care
  • Monitors patient status, labs, diagnostic results, on-call events, and significant condition changes, ensuring physician notification and plan-of-care updates
  • Facilitates case conferences, admissions, transfers, discharges, and interdisciplinary collaboration to ensure appropriate utilization of services and resources
  • Provides clinical leadership through supervision, orientation, quality oversight, risk reporting, and support of staff performance and regulatory compliance

Required Qualifications:

  • Current RN licensure in state of practice
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation

Preferred Qualifications:

  • Home Care experience
  • Current CPR certification or ability to complete within 90 days of hire

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $89,680 to $134,520 annually based on full-time employment. We comply with all minimum wage laws as applicable.
