We are currently seeking a dynamic and dedicated Permanent Patient Care Coordinator to join our team. This is a temp-to-hire role, providing an exciting opportunity to work within the fast-paced and ever-evolving manufacturing industry. The successful candidate will be responsible for coordinating all aspects of patient care, ensuring a smooth and efficient service that promotes patient health and wellbeing. This position requires a minimum of 5+ years of experience in a similar role.

Responsibilities:

As a Permanent Patient Care Coordinator, your primary responsibilities will include:

1. Coordinating and scheduling patient appointments, tests, procedures and consultations.
2. Serving as the primary point of contact for all patient care related issues.
3. Collaborating with healthcare professionals to plan and manage patient care.
4. Ensuring all patient records are accurately maintained and updated.
5. Providing a high level of customer service to patients, ensuring their needs are met in a timely and efficient manner.
6. Developing and implementing patient care plans in accordance with industry standards and regulations.
7. Conducting regular reviews of patient care plans to ensure they are effective and meet patient needs.
8. Providing education and guidance to patients and their families regarding their treatment and care.
9. Resolving any patient care issues or concerns in a professional and timely manner.

Qualifications:

The successful candidate for the Permanent Patient Care Coordinator role should possess the following qualifications:

1. A minimum of 5 years' experience in a similar role, ideally within the manufacturing industry.
2. Exceptional organizational skills with the ability to effectively schedule and manage patient appointments.
3. Strong interpersonal skills with the ability to communicate effectively with patients, their families, and healthcare professionals.
4. A high level of attention to detail and the ability to maintain accurate patient records.
5. A commitment to providing a high level of customer service.
6. Knowledge of industry standards and regulations relating to patient care.
7. The ability to work effectively as part of a team and independently.
8. A proactive approach to problem-solving and the ability to handle patient care issues effectively.
9. Proficiency in using computer software and systems relevant to the role.
