Requirements

• Master's degree or higher
• Clinical Experience (2+ years)
• 1+ years of experience in ANY of the following:
• Hospice
• Palliative Care
• Home Health
• ALL of the following valid licenses/certifications:
• Nurse Practitioner (NP) in Pennsylvania (PA)
• Registered Nurse (RN) in Pennsylvania (PA)
• ANY of the following valid licenses/certifications:
• American Heart Association Basic Life Support (AHA BLS)
• American Heart Association Advanced Cardiac Life Support (AHA ACLS)

Job Summary
We are seeking a compassionate and highly skilled Palliative Nurse Practitioner to join our dedicated healthcare team. Duties

• Conduct thorough patient assessments, including vital signs, symptom evaluation, and physiological examinations to determine care needs.
• Develop and implement individualized palliative care plans that address pain management, symptom control, psychosocial support, and advance care planning.
• Administer medications such as opioids and other symptom-relieving agents following best practices and regulatory standards.
• Coordinate with physicians, social workers, chaplains, and other healthcare professionals to ensure comprehensive patient support.
• Utilize electronic health record (EHR) systems like Epic and eClinicalWorks to document clinical encounters accurately and maintain detailed medical records.
• Provide education to patients and families about disease progression, treatment options, medication administration—including injections and IV infusions—and end-of-life care preferences.
• Participate in clinical research initiatives related to hospice and palliative medicine to advance evidence-based practices.
