Setting: Home Based-virtually rare occasion going into home.

Call/Weekends: Rotates on call for 1 week a month.

Position Overview:

Henry Ford Health is seeking an experienced, full-time Nurse Practitioner to join the Division of Hospice and Palliative Medicine. This role provides longitudinal palliative care for patients with serious illness, typically those with a prognosis of less than two years, with a manageable panel of approximately 200 patients. The NP will provide expert symptom management, advance care planning and goals-of-care discussions, coordinate transitions to hospice when appropriate, and collaborate with specialty providers while helping patients establish home-based primary care when needed. Working at the top of your license, you'll be part of a highly collaborative interdisciplinary team with daily interaction alongside home-based RNs and an excellent physician partner. The position offers the flexibility of a primarily remote schedule with travel to patient locations and monthly in-person interdisciplinary team meetings in Royal Oak (including training at the Josephine Ford Cancer Center). Previous Nurse Practitioner experience is required; new graduates will not be considered.

Qualifications:

  • Certification as a Nurse Practitioner
  • At least 5 years' experience with Palliative or Hospice
  • Specialty certification in hospice and palliative medicine (ACHPN for NP), preferred
  • 2 years' experience in HPM, preferred
  • BLS, DEA, CALS

Education & Training

  • Nurse Practitioner (NP): Completion of a Nurse Practitioner program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC)

Licensure

  • Current licensure as a Nurse Practitioner, the State of Michigan, or in the process of applying (proof of application required)

Certification

  • NP: National certification by a certification body recognized by the State of Michigan (e.g., ANCC, AANP)

Required Certifications

  • Basic Life Support (BLS) - required
  • Advanced Cardiovascular Life Support (ACLS) - required (may be obtained after hire)

DEA / Controlled Substance Licensure

  • NP: DEA registration required through credentialing; NPs do not hold a Michigan CSL and will use their Michigan license number during the application process
