PACE Day Center Supervisor LVN (Central Valley PACE - Stockton)

Has the responsibility for providing direct and indirect nursing care to assigned patients. Manages delivery of participants Activities of Daily Living (ADL's) assistance in the Day Center. Supervises personnel care attendants (PCAs). Performs clinical tasks within scope of responsibility, under the supervision of the Director of Center Operations RN.

Until our Stockton location is operational, the role will be based and trained at our Modesto center.

Schedule is Monday - Friday, 8:00am - 5:00pm with flexibility to work overtime.

Compensation: $33.79 - $35.48 an hour

Duties and Responsibilities

• Collects patient data and completes required forms with appropriate responses according to unit standards.
• Identifies patient's overt problems/needs and sets priorities; identifies problems requiring further referral and/or follow-up.
• Makes referrals to multidisciplinary support services.
• Communicates participant changes to team members.
• Implements Managed Care to conform to accepted practice standards.
• Provides treatments, procedures and overall care as prescribed and according to patient care standards, while acting within the scope of authority to practice.
• Demonstrates understanding of age-related characteristics and needs of patients served.
• Identifies emergency situations and independently initiates advanced therapy.
• Understands and demonstrates respect for patient rights and utilizes established mechanisms for management of ethical issues in patient care.
• Administers medications and/or monitors self-administration of medications by participants.
• Supervises personal care given by program aides, records patient care delivered as planned and any variation, with appropriate rationale.
• Makes and records observations related to impending or associated problems; implements nursing measures related to impending or associated problems.
• Recognizes unit problems and takes responsibility for documenting same; demonstrates active participation in QI processes; complies with PACE expectations to meet staffing demands based on patient care needs.
• Complies with policies addressing safe working conditions; monitors unsafe working conditions; recognizes inappropriate and/or ineffective patient care management, resolves issue/problem and completes written reports.
• Participates as a member of the Interdisciplinary Team (IDT). Attends staff meetings and takes part in participant care planning.
• Observes each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the RN/ Interdisciplinary Team.
• Ensures participants are oriented to the program and have opportunities to express their opinions about the care and services they receive.
• Assists Center Manager with effective planning, implementation, evaluation and improvement of facility processes.
• Supports general PACE maintenance activities to assure compliance with patient safety, infection control, quality assurance, mandated reporting, consent, and confidentiality policies/procedures.
• Administers and completes the TB testing process for all the prospective enrollees
• As needed in the clinic, provide direct care that includes drawing lab orders, blood sugar checks, wound care and/or identified follow up needed.
• Assist with quality measures.
• Other projects and duties as assigned.

Physical Demands

• Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds should be done with assistance as appropriate.
• Ability to exert maximum muscle force to lift, push, pull or carry patients.
• Must be able to hear staff/clients on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
• Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
• Must have high manual dexterity.
• Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.

Work Environment

• Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
• Subject to unpleasant odors.
• The noise level in the work environment is usually quiet to moderate, but may at times be noisy and crowded.

Education/Experience Requirements

Minimum Qualifications

• Valid CA Driver's License, acceptable driving record, and vehicle insurance.
• Ability to communicate skillfully, handle multiple priorities, maintain a warm and friendly demeanor, perform nursing duties, be flexible, innovative, focus on results and travel frequently or as required.
• Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties
• Skilled in teamwork, excellent communication, data entry, computer applications and electronic health records.
• Meet a standardized set of competencies for the specific position description established by Central Valley PACE and approved by CMS before working independently.

Education/Experience

• Graduate of an accredited school of professional nursing.
• Current unencumbered CA Vocational Nursing License.
• Current BLS CPR Card certified by American Heart Association.
• Minimum one (1) year of supervisory experience in a patient care setting.
• Minimum two (2) years LVN experience, within the last three (3) years.
• Minimum one (1) year experience working with the frail or elderly population.
