Job Summary:
The Outreach Coordinator/Administrative Assistant plays a key role in supporting both community engagement and internal program operations. This position is responsible for building and maintaining relationships with referral sources, increasing awareness of Achieve Beyond’s services, and supporting departments with provider and case management activities. The role combines external outreach with administrative and program support, ensuring accurate documentation, coordinated communication, and effective tracking of outreach and operational activities.
Essential Duties and Responsibilities:
- Uploading documentation into EMR system for providers and cases
- Complete weekly in person visits to community settings/referral sources such as pediatricians, daycares, etc.
- Establish relationships with referral sources local to our varying service locations
- Conduct regular outreach phone calls and distribute informational materials to prospective partners and families
- Represent organization through presentations, workshops, community meetings, and health fairs
- Monitor site visits and track incoming referrals to assess outreach effectiveness
- Utilize databases and applications for data entry, reporting, and tracking outreach metrics
- Identify, organize and attend community events and networking opportunities
- Meet weekly goals of outreach calls and visits to referral sources
- Assistance with staffing, intake, provider relations, and internal operations procedures.
- Assistance with quality assurance initiatives such as parent and provider surveys.
- Other duties as assigned by manager

Qualifications and Other Requirements:
- Office experience preferred
- Bachelor's degree preferred
- Excellent verbal, written, and interpersonal communication skills
- Strong organizational, time-management, and analytical skills
- Proficient in Microsoft Office (Word, Excel, Outlook)
- Ability to work in a fast-paced environment
Physical Requirements:
- Occasionally ascends/descends stairs going to, while at, or leaving work location or facilities traveled to for work events.
- Must be able to travel to and from assigned work events and outreach locations, including but not limited to driving personal vehicle, walking, and/or use of public transportation such as a bus or train.
- Occasionally moving and/or lifting objects at work weighing up to 20 pounds such as files or copy paper.
- Must be able to remain in a stationary position for prolonged periods of time up to 90% of the office day working on a computer/electronic device in office.
- The ability to observe details at close range (within a few feet of the observer).
- The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.
