Are you passionate about helping seniors and building meaningful community relationships? We are seeking a driven and compassionate Outreach Ambassador to expand community partnerships and connect older adults with healthcare services that support safe, independent living at home.

This role is ideal for candidates with healthcare outreach, senior living, relationship-based sales, or business development experience who thrive in a community-facing environment.

What You’ll Do

  • Develop and maintain referral relationships with hospitals, healthcare providers, senior organizations, and community partners

  • Educate prospective patients and families on available healthcare programs and services

  • Represent the organization at networking events, presentations, and outreach activities

  • Build strategic outreach plans to drive community growth and referrals

  • Collaborate with enrollment and clinical teams to ensure a smooth patient onboarding experience

  • Track outreach activities and referral data within CRM systems

  • Travel throughout the local Evansville community to build partnerships and engagement

Qualifications

3+ years of experience in community outreach, healthcare sales, business development, or relationship management
Experience working with seniors, elderly populations, or healthcare services preferred
Strong communication, presentation, and relationship-building skills
Microsoft Office and CRM experience required
Valid driver’s license and reliable transportation required
Ability to work independently and within a collaborative team environment

Preferred Qualifications

  • Bachelor’s degree preferred

  • Senior living or healthcare outreach background

  • Medicare/Medicaid knowledge

  • Referral development experience
