Duties

• Coordinate discharge planning with multidisciplinary teams to ensure seamless transition from hospital to home or community care settings.
• Manage care plans for diverse populations including pediatrics, geriatrics, and patients in acute care or outpatient clinics.
• Utilize electronic medical record (EMR) systems efficiently to document patient progress, treatment plans, and clinical notes accurately.
• Provide patient education on injury prevention, rehabilitation exercises, and health maintenance strategies to empower self-care.
• Collaborate with physicians and other healthcare professionals to optimize treatment outcomes and adapt care strategies as needed.
• Participate in home & community care initiatives to extend physiotherapy services beyond clinical settings and support patient independence.
• Apply specialized techniques such as trigger point therapy and orthopedic physiotherapy to address complex musculoskeletal issues.
• Engage in continuous professional development by staying current with medical terminology updates, physiotherapy advancements, and best practices in inpatient, outpatient, hospital, or home health environments.

Skills
