The primary role of the nurse coordinator is to ensure seamless coordination of patient care across the continuum. This includes clinical practice, care coordination, education, quality improvement, with significant latitude for exercising professional judgement. The Nurse Coordinator is an integral member of the multidisciplinary team providing developmentally appropriate care to patients and families within all Pediatric Treatment Centers. The Nurse Coordinator acts as liaison between patient, family, provider, clinical staff, pharmacy, and scheduling and registration personnel to ensure safe and efficient patient flow.\n \nEEO/AA/Disability/Veteran\n \nResponsibilities \n\n 1. Professional Practice:\n 1.1 The Pediatric Nurse Coordinator is the lead member of the health care team coordinating all aspects of patient care for the specific specialty.\n 2. Education\n 2.1 Provides patient and family education on appropriate medical condition, including signs, symptoms , treatment regimens, side effects, outcomes expected, and potential adverse reactions or outcomes. Including manangement of condition, promoting optimal level of health.\n 3. Multi-Disciplinary Collaboration\n 3.1 Functions as a key member of the multi-disciplinary team coordinating all aspects of patient care across the continuum\n 4. Quality Improvement\n 4.1 Participates in data collection related to clinical and operational efficiencies.\n \nQualifications\n \nEDUCATION\n \nRegistered Nurse , Bachelor's Degree in Nursing preferred, Connecticut RN license.\n \nEXPERIENCE\n \n2-3 years' experience in nursing and have knowledge of infusion therapy. Experience in ambulatory setting preferred.\n \nLICENSURE\n \nLicensed as a Registered Nurse from the State of Connecticut. APHON certification preferred not required.\n \nSPECIAL SKILLS\n \nExcellent interpersonal and organizational skills. Ability to assert oneself, advocate for patients, and work independently with minimal supervision. Ability to manage multiple priorities. Ability to develop the appropriate knowledge, with instructions from staff and outside resources, regarding bleeding, clotting and vascular disorders, to develop and maintain competence as a nurse coordinator in this highly specialized area. Computer experience and knowledge of word processing and data entry systems. Ability to learn to work effectively with an electronic medical record required. Must be able to communicate effectively with all members of the healthcare team, patients and families. Must value and execute "patient-centered model of care."\n \nPHYSICAL DEMAND\n \nMust be flexible to meet the demands of the specialty, outpatient clinic session, specialty and patient/family needs. May need to travel to other sites for clinics.\n \nADDITIONAL DETAILS\n \nThe RN is responsible for providing clinical care, education and coordination for pediatric and adult patients with bleeding and clotting disorders. This role requires strong communication skills, ability to work collaboratively within a multidisciplinary team and supporting patients across the lifespan. Travel to multiple sites is required (Trumbull and Orange)\n \nThe primary role of the nurse coordinator is to ensure seamless coordination of patient care in order to address bleeding and clotting issues to ensure best possible outcomes. This work includes clinical practice, care coordination, patient and staff education, quality improvement, and has significant latitude for exercising professional judgement when experience is attained. The Nurse Coordinator is an integral member of the multidisciplinary team providing developmentally appropriate care to patients and families of all ages, but with a concentration on adult care, with primarily bleeding but also clotting and vascular disorder. The Nurse Coordinator acts as liaison between patient, family, provider, clinical staff, pharmacy, and scheduling and registration personnel to ensure safe and efficient patient flow.\n \nThe RN coordinator functions as an effective member of a multi-disciplinary lifespan team by displaying respectful communication, deferring to the expertise of team members with complementary roles, and by upholding professional standards of a health care team member.\n \nAdditional Information \n \n2+ years of acute RN experience required.
