The Office Nurse, RN is a vital member of the primary care team, responsible for providing skilled nursing care, patient education, and clinical triage within our fast-paced health center. This role focuses on promoting health and preventing disease in a diverse patient population, working collaboratively with physicians, mid-level providers, and medical assistants to ensure efficient and high-quality patient flow and chronic disease management.
ESSENTIAL FUNCTIONS:
Direct Patient Support:
Clinical Procedures: Administer medications, immunizations (including complex pediatric and adult schedules), therapeutic injections and other treatments as ordered by the provider.
Point-of-Care Testing: Perform and track results for necessary in-house lab tests (e.g., POC glucose, strep, urinalysis) and ensure accurate specimen collection for external labs.
In-Room Support: Assist providers with minor in-office procedures, sterile technique setup and wound care.
Documentation: Accurately record all patient assessments, interventions, medications, and education in eCW in compliance with charting standards.
Triage, Education and Chronic Care Management:
Telephone Triage: Assess and prioritize acute patient symptoms over the phone, determining the urgency of care required, providing necessary advice, and scheduling appointments appropriately.
Patient Education: Provide comprehensive, culturally sensitive patient education on new diagnoses (e.g., diabetes, asthma, hypertension), medication usage, lifestyle changes, and self-management techniques.
Care Coordination: Manage patient registries for chronic illnesses, ensuring patients receive preventative screenings, necessary labs, and follow-up care in a timely manner.
Test results: Review and communicate routine and abnormal lab/imaging results to patients under the direction of the provider.
Operational and Quality Improvement:
Patient Flow: Oversee and manage the clinical patient flow, ensuring smooth transitions between check-in, rooming, provider visit, and discharge.
Supply Management: Monitor and manage inventory of clinical supplies, medication, and vaccines, ensuring proper storage and handling according to OHSA and clinical guidelines.
Compliance: Maintain strict adherence to infection control protocols, safety regulations, HIPAA privacy standards, and all organizational policies.
Quality Metrics: Participate in quality improvement initiatives, contributing to the clinic's efforts to meet or exceed targets for clinical quality and patient satisfaction.
