Make outbound calls to healthcare facilities using contact lists provided by the Field Outreach Representative
Follow up with facilities within 24–72 hours after initial in-person visits
Confirm and update contact information for key departments, including:Social Work
Case Management
Discharge Planning
Schedule and confirm meetings for the agency owner (phone, virtual, and in-person meetings)
Send professional emails for introductions, appointment confirmations, and follow-up communication
Answer incoming calls and appropriately respond to or route messages
Maintain and update CRM systems and Google Sheets with accurate notes, call logs, and appointment details
Perform general clerical and administrative duties, including:Filing client and staff documents
Organizing digital records
Data entry
Assist with intake-related tasks and additional administrative support as needed
Conduct light research to verify healthcare facility contacts and department information
Coordinate the mailing of marketing postcards through the United States Postal Service (USPS)
Monitor and respond to inbound inquiries from sources such as Google Business Profile, referrals, and phone calls
Purpose
