Occupational Therapist (OT) – Skilled Nursing Facility
Location: Hobe Sound, FL
We’re seeking an experienced Occupational Therapist (OT) to join a reputable Skilled Nursing Facility in Hobe Sound, FL. This is a full-time, permanent role for an OT who is comfortable working in a SNF / long-term care environment and treating a geriatric and post-acute population.
Position Overview
The Occupational Therapist will evaluate residents and provide skilled therapy services focused on improving independence with activities of daily living, functional mobility, and safety.
Key Responsibilities
- Conduct occupational therapy evaluations and develop individualized treatment plans
- Provide skilled OT services to short-term rehab and long-term care residents
- Address ADLs, functional mobility, cognition, and adaptive equipment needs
- Collaborate with PTs, SLPs, nursing staff, and physicians as part of the care team
- Document evaluations, treatments, and progress in compliance with facility and regulatory standards
- Participate in care planning and discharge planning as appropriate
Qualifications
- Active Florida Occupational Therapist license
- Prior experience in a Skilled Nursing Facility or Long-Term Care setting required
- Strong documentation and time-management skills
- Ability to work independently in a fast-paced clinical environment
- Experience with geriatric and post-acute rehab patients preferred
