Location: Fort Dodge, Iowa (Onsite / Home Health)

Hours: 36–40 Hours per Week
The Occupational Therapist (OT) – Home Health is responsible for providing in-home therapy services to patients to help them regain independence in daily living activities.
This role focuses on improving functional abilities, enhancing quality of life, and delivering patient-centered care in a home-based setting.
Must-Have Qualifications
• Minimum 1–2+ years of Occupational Therapist experience (Home Health preferred)
• Valid Occupational Therapist License – Iowa (Required prior to start)
• BLS Certification (AHA – Required)
• Degree in Occupational Therapy (OT or equivalent)
• Strong knowledge of rehabilitation and ADL training
• Valid Driver's License (Required for home visits)
Preferred Qualifications
• Experience in home health or community-based care settings
• Familiarity with EMR systems
• Strong patient education and communication skills
Work Environment
• Home Health / Patient Home Settings
• Onsite / Travel-based role within Fort Dodge, IA
• Independent field-based clinical environment
Additional Requirements
• Must have reliable transportation for home visits
• Must be flexible with day shifts and scheduling needs
• Ability to manage independent caseloads
• Must comply with all clinical and regulatory standards
