Our client is seeking a dedicated Occupational Therapist (OT) to join our in-house rehabilitation team. This is a great opportunity to work in a collaborative, patient-focused environment where you can make a meaningful impact on residents’ independence and quality of life.

Your Role
- Evaluate residents and develop individualized treatment plans based on medical diagnoses
- Implement therapy programs to improve functional independence and daily living skills
- Monitor and document resident progress through daily notes, progress reports, and re-certifications
- Maintain accurate and compliant documentation of care services
- Supervise and provide clinical guidance to Occupational Therapy Assistants (OTAs)
- Participate in care plan meetings and interdisciplinary collaboration
- Assist with departmental projects, reporting, and data tracking as directed
- Ensure all care is delivered in compliance with state, federal, and facility guidelines
Qualifications
- Graduate of an accredited Occupational Therapy program
- Active New York State OT License required
- Strong clinical, documentation, and communication skills
- Experience in skilled nursing or rehab setting preferred
- Team-oriented with a commitment to high-quality patient care
