Position Summary:
Provides individualized occupational therapy for inpatients including evaluation, treatment planning, goal setting, implementation of treatments and education of the patient and family.
Manages patient care
Completes evaluation same day as initial visit.
Evaluates objectively, based on patient diagnosis.
Provides goal-oriented patient treatment which is appropriate to the prognosis and physical/mental status.
Evaluations/treatments are aggressive and appropriate.
Introduces self and explains interventions to patients.
Coordinates and collaborates effectively with other health care disciplines to serve identified patient needs.
Participates in staff and patient/family conferences as needed.
Documents Patient Care
Short term and long term goals should be objective and functional with time frames.
Plans of care includes interventions with frequency and duration.
Includes objective and functional long and short-term goals in the plan of care. The goals also include time frames, treatment type, frequency and duration with revisions as necessary.
Documents objective clear, concise and up to date information about the treatment and the patient's response in the medical record according to the departmental policies.
Records treatment sessions accurately on patient information sheet, outpatient summary sheet and charge sheet.
Complies with all insurance requirements and requests for patient certification/recertification.
Educates the patient/family and plans for discharge in conjunction with team
Provides and documents patient, family and other health care providers education regarding the plan of care, treatments, goals, discharge plans, safety techniques and home instruction.
Recognizes patient/family needs for teaching and implements educational plan based on identified needs.
Works with the team and other health care providers to provide appropriate discharge planning.
Position Qualification:
Licensed as an Occupational Therapist in the State of Tennessee.
