Responsibilities
Conduct comprehensive patient evaluations in the home setting
Develop and implement individualized treatment plans
Address activities of daily living (ADLs) and instrumental ADLs
Provide therapeutic interventions to improve functional independence
Perform home safety assessments and recommend adaptive equipment
Educate patients and caregivers on compensatory strategies
Coordinate care with physicians and interdisciplinary team members
Complete accurate and timely documentation
Ensure compliance with Medicare and home health regulations
Qualifications
Active Occupational Therapist (OTR/L) license
Home Health experience preferred but not required
Strong assessment and care planning skills
Valid driver’s license and reliable transportation
Ability to work independently in the field
Excellent communication and organizational skills
